Check or money order should be made payable to Vital Statistics. Call or visit the Web site to verify current fees. All requests for vital records must be accompanied by a photo copy of the requestor's drivers license or other official photo identification, and the signature of the requestor.

Comments: Birth in Florida less than 100 years old are not public records and are only available to the person listed on the certificate, their parents or legal guardian, or a legal representative. If the individual named on the certificate is deceased, their birth certificate is also available to the registrant's spouse, child, grandchild, or sibling when the request is accompanied by a copy of their death certificate.

Comments: Anyone may order a Florida death certificate, but the cause of death is confidential for a period of 50 years following the death and will only be released to the decedent's spouse, parent, and (if of legal age) child, grandchild, or sibling. If you don't state your relationship to the individual, you will be issued a certified copy excluding cause of death information.

Comments: There are no restrictions on obtaining marriage certificates from the state of Florida. Marriage records filed in the office from 1972 through the present will also include the marriage application. With your request, please include the full names of the husband and wife, including wife's maiden name, the date of marriage (at least the year), and the city or county where the marriage license was issued.
