members and legal representatives. If you are not a family member you can
send your request with a notarized letter of consent signed by a family
member. With your request, include as much as you can of the

being requested, date of birth, place of birth (city or county), father's full name,
(last, first, middle), mothers full name, including her maiden name, your
relationship to the person whose certificate is being requested, your
purpose for needing the copy, your daytime telephone number with area code,
your handwritten signature and complete return mailing address. * Some earlier records for the city of Portland (from approximately 1880) and for a few individual counties are available from the Oregon State Archives, 1005 Broadway, NE, Salem, OR 97310. Some of the indexes to the records are available for searching online. You can also search

The State Office issues a summary document called a divorce certificate. A divorce certificate does not have all the information contained in the decree so is not as useful for genealogists.
