Event: Birth

Dates: From 1905

Cost of copy: $12.00

Comments: Access to birth records in Utah is restricted to the person named on the certificate and immediate family members only. With your request include as much as you can of the following: the name on the birth record being requested, date of birth, place of birth (city or county), father's full name, (last, first, middle), mothers full name, including her maiden name, your relationship to the individual named on the certificate, the reason for your request, your daytime telephone number with area code, your mailing address and your signature.

* If the birth occurred from 1890-1904 in Salt Lake City or Ogden, contact the city health department (Salt Lake City Vital Records,  Ogden Vital Statistics)

* For records elsewhere in the State from 1898-1904, write to the County Clerk in county where birth occurred.


Event: Death

Dates: From 1905

Cost of copy: $9.00

Comments: Access to death records in Utah is restricted to the person named on the certificate and immediate family members only. With your request include as much as you can of the following: the name on the death record being requested, date of death, place of death (city or county), your relationship to the individual named on the certificate, the reason for your request, your daytime telephone number with area code, your mailing address and your signature. Death certificates from 1905-1950 are also available from the Utah State Archives for a fee of $0.25 per page.

Comments: The State Vital Records Department only offers short-form certified copies - the information is abstracted from the marriage record, rather than it being an actual copy of the original record. If the marriage occurred prior to 1978 or you would like a photocopy of the original marriage record, you will need to contact the County Clerk in the county where the license was issued to obtain a certified copy. With your request, include as much as you know of the following: full names of bride and groom (including bride's maiden name), the date of marriage, the place (city, county) of marriage, your relationship to the people named on the certificate, the reason for your request, your daytime telephone number with area code, your mailing address and your signature.

Cost of copy: $9.00

Comments: The State Vital Records Department only offers short-form certified copies - the information is abstracted from the divorce record, rather than it being an actual copy of the original record. If the divorce occurred prior to 1978 or you would like a photocopy of the original divorce decree, you will need to contact the court in the county where the decree was issued to obtain a certified copy. With your request, include as much as you know of the following: full names of husband and wife, the date of divorce, the place (city, county) of divorce, your relationship to the people named on the certificate, the reason for your request, your daytime telephone number with area code, your mailing address and your signature.
