Event: Birth

Dates: From January 1917

Cost of copy: $5.00

Comments: Vital records in West Virginia are available only to family members (parents, spouses, siblings, grandparents, children, grandchildren and in-laws). With your request include as much as you can of the following: the name on the birth record being requested, date of birth, place of birth (city or county), father's full name, (last, first, middle), mothers full name, including her maiden name, your relationship to the individual, the purpose for your request, your daytime telephone number with area code, your mailing address and your signature.

Event: Death

Dates: From January 1917

Cost of copy: $5.00

Comments: Vital records in West Virginia are available only to family members (parents, spouses, siblings, grandparents, children, grandchildren and in-laws). Comments: Marriage records are available from the Clerk of County Court in the county where the marriage occurred.  With your request, include as much as you know of the following: full names of bride and groom (including bride's maiden name), the date of marriage, the place (city, county) of marriage, your daytime telephone number with area code, your mailing address and your signature.  


Event: Divorce

Dates: From January 1, 1968

Cost of copy: $5.00

Comments: Certified copies are not available from the State Office. Divorce records are available from the Clerk of Circuit Court, Chancery Side, in county where divorce was granted. With your request, include as much as you know of the following: full names of husband and wife, your relationship to the individual, the reason for your request, your daytime telephone number (with area code), your mailing address and your signature.
