A stamped, self-addressed business size (#10) envelope should also be included with the request. Additional copies of any record ordered at the same time are only $2. Event: Birth

Dates: Fairly complete from October 1907; scattered records going back as far as 1814

Cost of copy: $12.00

Comments: Wisconsin State Law states that only the person named on the record, the immediate family of that person or anyone with written permission from the person named on the record or the person's family may get a "certified" copy of a birth certificate. A "non-certified" copy of a birth certificate, however, is available to anyone who applies. With your request include as much as you can of the following: the name on the birth record being requested, date of birth, place of birth (city or county), father's full name, (last, first, middle), mothers full name, including her maiden name, your relationship to the individual, the purpose for your request, your daytime telephone number with area code, your mailing address and your signature.

* If you have incomplete information (not sure of county, date, name of parents, etc.), the Wisconsin Vital Records Office will undertake a genealogy search. The $12 fee covers a search period of several years - for one single county. This page describes both the search periods and the dates for which they have records for each county.

* If you know the county of birth you will usually have the best luck with records prior to October 1907 by contacting the county Register of Deeds office.
