What You Need to Know:
Check or money order should be made payable to Idaho Vital Statistics. Personal checks are accepted. Certificate requests can also be accepted by fax for an additional $10.50 in special handling fees. Call or visit the Web site to verify current fees. Applicants must provide a copy of both sides of their drivers license or other government photo ID, or a copy of two other forms of identification with a signature.

Idaho Birth Records:

Dates: From July 1911

Cost of copy: $13.00

Comments: Birth certificates are legally confidential in Idaho for 100 years, making them only available to immediate family members (no step-relatives, in-laws, great-grandparents, etc.). Proof of relationship may be required.

With your request, include as much as you can of the following: the name on the birth record being requested, date of birth, place of birth (city or county), father's full name, (last, first, middle), mothers full name, including her maiden name, your relationship to the person whose certificate is being requested, your purpose for needing the copy, your daytime telephone number with area code, your handwritten signature and complete return mailing address.

Idaho Death Records:

Dates: From July 1911

Cost of copy: $13.00

Comments: Death certificates are legally confidential in Idaho for 50 years, making them only available to immediate family members for deaths during that time frame.

With your request, include as much as you can of the following: the name on the death record being requested, date of death, place of death (city or county), your relationship to the person whose certificate is being requested, your purpose for needing the copy, your daytime telephone number with area code, your handwritten signature and complete return mailing address.
