Check or money order should be made payable to Kentucky State Treasurer. Personal checks are accepted. Call or visit the Web site to verify current fees. Certificates may be ordered in person or by sending in a written request. For an extra $5 processing fee, certificates may also be ordered by telephone or fax with a major credit card.

Dates: From July 1, 1911

Comments: With your request include the full name of the deceased, the month, day and year of death, the county of death and your address and daytime telephone number.
