D.C. Vital Records - Births, Deaths & Marriages

All requests for birth and death record must be accompanied by a photo copy of the requestor's identification.

Dates: From August 1874

Cost of copy: $23.00 long-form, $18 short-form

Comments: State law restricts access to birth records less than 100 years old to the registrant and immediate family members. With your request, include as much as you can of the following information: full name of the individual, their date and place of birth, full name of father, full maiden name of mother, your relationship to the person whose certificate you are requesting, your address and daytime phone number.
