With your request, include as much as you can of the following information: full name of the individual, their date and place of birth, full name of father, full maiden name of mother, your relationship to the person whose certificate you are requesting, your address and daytime phone number. Death Records: Dates: From August 1874 (some available from 1855, but no death records were filed during the Civil War) Cost of copy: $18.00 Comments: State law restricts access to the registrant and immediate family members for a period of 50 years. With your request, include as much as you can of the following information: full legal name of the deceased, date and county of death, sex, date of birth, race, name of spouse, name of parents, the reason for your request and your relationship to the person named on the death certificate.
